What is your cancellation policy?

Clients must cancel within 24 hours or a $75 cancellation fee will be charged. Clients who no call, no show will be charged a $75 cancellation fee.

What insurance do you accept?

Kulima Therapy is paneled with BCBS, Select Health, PEHP, EMI Health, Cigna, and Aetna.

How much are your services?

Individual sessions are $145 per session. Supervision is $125 per meeting. A sliding scale is available for uninsured individuals. BIPOC and LGBTQIA+ individuals who may need this sliding scale are prioritized. Workshop prices vary. Trainings are $200 per hour.

Our office is located at 150 S 600 E #8C in Salt Lake.

Do you have openings for supervisee’s?

Currently accepting supervisees and/or interns to provide contract supervision.

What is the good faith estimate?

All those receiving mental health services who are under insured, not insured, or do not wish to use their medical insurance benefits for services, are entitled to receiving a Good Faith Estimate (GFE). This also includes those who do not plan to submit Superbills to their insurance company. This GFE allows for the individual to know what charges to expect while paying for out-of-pocket or out-of-network services over the span of 12 months, or 50 therapy sessions. Any and all changes to this amount will be updated in writing and given to the individual.
